1. Nomination forms can be downloaded as an Adobe PDF document. Nominations may be made by a Section or Branch, a standing ASCE committee or an individual ASCE member. Documentation of the individual's contributions and its positive impacts must accompany the nomination form.
   
   
2. Four copies of the nomination package, including the nomination form and supporting information must be received at ASCE headquarters no later than Oct. 1.
